Teeth Whitening

Stains happen! That cup of coffee you just can’t start your morning without, that glass of red wine you like to enjoy with dinner, and even your mom’s famous spaghetti sauce all leave trace stains on your tooth enamel that, over the years, can result in a dull and discolored smile. Luckily, removing those stains can be as easy as a visit to the dentist. For a dramatically whiter smile in a little over an hour, in-office whitening can achieve results using high-strength whitening agents and a special activation light. For patients who would prefer the convenience of eradicating tooth stains at home, at-home whitening trays are custom-fitted, easy to wear, and show results in as little as three days, with full results in one to two weeks.

It is very rare to come across a patient with a naturally bright, white smile. There are simply too many factors that can affect the color of the teeth, either creating unsightly stains or giving the entire tooth a dull, dingy appearance. For many patients who are dissatisfied with the color of the teeth, teeth whitening can drastically improve tooth color to create a brighter, whiter smile. However, patients should understand that teeth whitening will not produce satisfactory results for all. What many people may not realize is that not all tooth discoloration is the same, and, therefore, different types of tooth discoloration will require different treatment.

Types of Tooth Discoloration

When patients come in for tooth discoloration treatment, it is important to determine what type of discoloration is present so that appropriate treatment options can be discussed. When we discuss the different types of tooth discoloration, we are essentially discussing which part of the tooth has been discolored, either the surface of the tooth (the outer layer known as the enamel) or the underlying layer of the tooth (the dentin). Below are some examples of the different types of tooth discoloration:

  • Extrinsic discoloration: Extrinsic stains are those that are present on the surface of the tooth. These are more superficial stains and discoloration that do not extend beyond the outer layer of the tooth’s enamel. Examples of extrinsic discoloration include stains or discoloration from food and beverages, the use of tobacco products, aging, or poor oral hygiene habits. Extrinsic discoloration can cause brown spots, white or yellow streaks, or pit marks of brown or yellow stains.
  • Intrinsic discoloration: Intrinsic discoloration that is present on the dentin layer of the tooth is more difficult to treat than extrinsic discoloration. These stains are described as more permanent forms of discoloration and can be caused by factors such as the use of certain medications, an over exposure to fluoride, aging, and traumatic injury to the teeth. Intrinsic discoloration generally gives the teeth an overall gray or dingy appearance.

Treating Tooth Discoloration

Each type of tooth discoloration responds best to different types of tooth discoloration treatment. Below are some treatment options that may be considered for the two primary types of tooth discoloration:

  • Extrinsic discoloration: Because this type of discoloration only lies on the surface of the teeth, it responds very well to teeth whitening treatment. With professional teeth whitening, surface stains can be eliminated and the overall color of the teeth can be improved by an average of eight shades. Unless additional cosmetic flaws are present, teeth whitening should be the only treatment required for patients experiencing extrinsic discoloration.
  • Intrinsic discoloration: This underlying discoloration is not likely to respond to the bleaching components in teeth whitening gels. Patients with intrinsic stains should consider cosmetic treatments that will conceal areas of discoloration. These treatments include dental bonding, dental crowns, and porcelain veneers.

Teeth Whitening Cost

In-office tooth whitening procedure, the cosmetic dentist applies a bleaching gel that contains hydrogen peroxide to the patient’s teeth. The procedure calls the patient to sit through two consecutive sessions each lasting about 15-30 minutes. Throughout the sessions an ultraviolet light (laser light) is used to help speed up the process. This procedure offers the fastest results for teeth whitening but it also the most costly with an average price of $700 per visit.

The main options for take home whitening kits are take home whitening trays and whitening strips. Whitening trays also called teeth bleaching kits can only be obtain from a licensed dentist. These kits can provide the best results because they can be applied to one’s teeth over a prolonged period of time. The results can last for a period of up to six months. These kits contain custom fitting trays with the bleaching gel inside or in syringes that can be added before its used. The trays are made from a flexible material (usually plastic) and they protect the gums from becoming irritated by the bleaching gel. Popular brands include Zoom! Weekender Kit, Nite White Turbo, NiteWhite and DayWhite ACP by Discus Dental and Ultradent’s Opalescence. These products can cost up from $50 to $400.

What Are the Most Common Side Effects of Teeth Whitening?

Most patients can expect to experience some side effects during or after their teeth whitening treatments. These more common side effects include:

  • Heightened Tooth Sensitivity: The bleaching agents used in teeth whitening must penetrate the outer enamel layer of your teeth. Because of this, many patients experience heightened sensitivity to hot, cold, sweet, and sour foods for anywhere from a few hours to a few days after their treatments.
  • Discomfort or Pain: In addition to heightened sensitivity, some patients may also experience some additional temporary discomfort or mild pain as bleaching agents penetrate the teeth and come close to the pulp and tooth root.
  • Gum Irritation: Your dental team will do their best to avoid any whitening gel coming in contact with your gums, but at times this cannot be avoided. Bleaching agents can irritate delicate gum tissue, causing temporary redness or inflammation.

Can Teeth Whitening Cause Permanent Side Effects?

Most of the side effects of teeth whitening treatments are very mild and temporary, lasting a few days at most, but there are some potential side effects that can have longer-lasting or even permanent consequences. These issues tend to be caused by poor cosmetic dentistry or a lack of communication between a patient and cosmetic dentist. Some of these side effects include:

  • Tooth Discoloration: Many patients assume that, if one tooth whitening treatment is good, multiple treatments are even better. In fact, excessive teeth whitening can dull the teeth. By wearing too heavily on the sensitive enamel coating, over bleached teeth can take on a dull yellow or gray appearance. If caught early enough, a patient may be able to repair this damage with enamel-strengthening mouthwash or toothpaste. But if the enamel has suffered too much damage, dental bonding or porcelain veneers may be the only way to improve the appearance of the teeth.
  • Nerve Damage: Most patients experience some discomfort and increased sensitivity during the teeth whitening process, as the bleaching agents seep into the enamel in order to whiten the teeth. For the majority of these patients, that discomfort wears off quickly. For others, especially patients who already have sensitive teeth or thin enamel, that discomfort can last much longer or even cause permanent nerve damage. Be sure to discuss any preexisting tooth sensitivity with your dentist before getting in the dentist’s chair for a teeth whitening treatment – he may be able to use an alternative method or suggest a different treatment altogether in order to preserve the integrity of your teeth.

What Is Zoom!® Tooth Whitening?

Zoom!® is a bleaching process that lightens discoloration of enamel and dentin.

What Causes Tooth Discoloration?

There are many causes. The most common include aging and consumption of staining substances such as coffee, tea, colas, tobacco, red wine, etc. During tooth formation, consumption of tetracycline, certain antibiotics or excessive fluoride may also cause tooth discoloration.

Who May Benefit from Tooth Whitening ?

Almost anyone. However, treatment may not be as effective for some as it is for others. Your dental professional can determine if you are a viable candidate for this procedure through a thorough oral exam, including a shade assessment.

Do Many People Whiten Their Teeth?

More people than you might imagine. A bright, sparkling smile can make a big difference for everyone. The Zoom!® Whitening System makes it easier and faster than ever before.

Is Whitening Safe?

Yes, extensive research and clinical studies indicate that whitening teeth under the supervision of a dentist is safe. In fact, many dentists consider whitening the safest cosmetic dental procedure available. As with any tooth whitening product, Zoom!® is not recommended for children under 13 years of age and pregnant or lactating women.

How Long Do the Results Last?

By following some simple post whitening care instructions, your teeth will always be lighter than they were before. To keep your teeth looking their best, we recommend flossing, brushing twice daily with Zoom!® Whitening Toothpaste, and occasional touch-ups with Zoom!® take-home gel. These are professional formula products designed specifically to keep your teeth their brightest. They are available only through your dental professional.

Is There Any Difference Between Over-The-Counter Tooth Whiteners and Zoom!®?

Yes! Consulting your dental professional is always the first step to whitening your teeth safely. Plus, over-the-counter tooth whitening strips, brush-on gels and toothpastes take weeks or months to whiten your teeth just a few shades. Zoom!® is clinically proven to whiten your teeth an average of eight shades in one hour or six shades in just one weekend.* Only a dentist can get your teeth this white, this fast.

How Does the Zoom!® In-Office System Work?

The Zoom!® light-activated whitening gel’s active ingredient is Hydrogen Peroxide. As the Hydrogen Peroxide gel is broken down, oxygen enters the enamel and dentin, bleaching colored substances while the structure of the tooth is unchanged. The Zoom!® light aids in activating the Hydrogen Peroxide and helps it penetrate the surface of the tooth. A study has shown that use of the Zoom!® lamp increases the effectiveness of the Zoom!® gel by 33% or more, giving an average improvement of eight shades*.

What Does a Patient Experience During the Zoom!® In-Office Procedure?

During the procedure, patients may comfortably watch television or listen to music. Many actually fall asleep. Individuals with a strong gag reflex or anxiety may have difficulty undergoing the entire procedure.

Are There Any Side Effects?

Some people experience temporary increased tooth sensitivity to cold during treatment. These symptoms disappear within 1-3 days after interruption or completion of the treatment.

How Is Exposure to The Zoom! ® Light Controlled?

The Zoom!® procedure requires that all soft tissue in and around the mouth be protected. The clinician is required to monitor the procedure to insure any exposed skin or tissue is isolated and protected. Protective eyewear is required during the procedure. Light sensitive individuals including those undergoing PUVA (Psoralen+UV Radiation) therapy or other photo-chemotherapy, as well as patients with melanoma, should not undergo the Zoom!® In-Office procedure. Also patients taking any light sensitive drugs or substances, whether over-the-counter, prescription or homeopathic, should consult their primary physician prior to procedure. Anyone who is not eligible for Zoom!® In-Office for these reasons may be an excellent candidate for Zoom!® Take-Home.

How Long Does Zoom!® In-Office Whitening Take ?

The complete procedure takes just over an hour. The procedure begins with a preparation period followed by one hour of bleaching. A five minute fluoride treatment completes the procedure (a cleaning is recommended prior to the actual Zoom!® Whitening session).
